The Child Observation Log is a Google Sheets documentation tool designed to help you track your child’s symptoms, behaviors, and observations over time and see when they occur in relation to parenting time and transition days.

Log observations such as physical symptoms, emotional changes, behavioral concerns, social behaviors, and more. Each entry can include the date, parenting time, transition timing, observation category, subcategory, observer, notes, and supporting references.

The built-in summary helps organize your entries so you can identify patterns, including observations occurring before transitions, on transition days, or after transitions to either parent’s home.

This template is created in Google Sheets and is designed to keep your documentation organized in one easy-to-reference place. Simply make your own copy and begin entering your information.
